Every type of seizure and now tremors too

My 5 year old 70 lb dog has had seizures for a little over a year now. It started with tonic clonic seizures and about a week later she began having focal seizures as well. Sometimes they’re generalized, sometimes focal, sometimes they start out as focal and turn into general, it’s a little bit of everything. They tend to happen in clusters.

She's been on phenobarbital since the beginning and then Keppra was added on when her seizures started happening more frequently again. Even on the meds she usually has a seizure once a month. Now she occasionally has tremors (?) too. A few minutes of head nodding or very mild jaw trembling.

She's definitely taken a cognitive hit from all of this and she was a zombie in the early days but she's doing well now, all things considered. She's still happy and playful and otherwise healthy.

However, my vet never mentioned any other tests like bile acid or thyroid and I’m wondering if I should take her to a specialist. I can’t afford an MRI and my vet recommended against it anyway because he said it wasn’t worth the cost since the treatment would ultimately be the same. The only tests he said are necessary are the CBC blood panels and the phenobarbital levels taken 6 hours after a dose. Should I insist on more tests and if so, which ones?

(Current medications: phenobarbital 97.2 mg 2x a day + levetiracetam 750 mg 3x a day)

you know your pup and if you think they need additional tests then go for it. My vet said she thought everything was routine with my pup, until I pointed out in the journal I keep about his seizures that the only factor that was always the same was that he was sleeping. So we then did additional tests and have found that his insulin is high and his glucose is low. So now we are look at the possibility of a mass on his pancreas . If you think you need tests don't be afraid to push for them.

My dog is the same way. Almost all of her seizures are when she's sleeping or trying to sleep. The vet's aware of this but never commented on it. I'll ask about those tests, thank you!
